TREATMENT
Below you can find important information regarding mental health treatment in New York and New Jersey. Services are provided in person or via telehealth. Please read the information below regarding the Clinical Management Consultants treatment center at Freedom from Fear.
The independent treatment center located at Freedom from Fear, Clinical Management Consultants Inc. consists of psychiatrists (medical doctors who can provide medication), social workers, and other licensed mental health professionals. All of the staff has experience in treating all of the major anxiety disorders (including generalized anxiety disorder, social anxiety disorder, panic disorder, obsessive-compulsive disorder, and post-traumatic stress disorder) as well as depressive illnesses. All of the therapists are also well trained in Cognitive Behavioral Therapy.

You can download, print and complete the cover page. Afterward, please email a scanned copy.
If you have Adobe Acrobat (or a similar PDF editor) you can complete the form online and email the Intake Request form back to us.
(2) A picture of your insurance card (front and back) must be included in the email with the cover page.
We accepts most insurance, however we do not accept Medicaid or Medicaid-based insurances.
Like adults, many children and adolescents are faced with obstacles throughout their developmental years. These bumps along the way may result in strong feelings, emotions, and behaviors that affect their lives and their loved ones around them. Parents, caregivers, siblings, extended family members, and friends are often worried when a child or adolescent is having difficulty coping with a certain situation, is feeling hopeless or sad, is having a hard time sleeping, attending school, or getting along with a peer or family member. Although these hardships may sometimes seem impossible to overcome, there is hope and the good news is, they can be treated.
Anxiety and Depression Treatment Center at Freedom From Fear - Located at Freedom From Fear's Staten Island headquarters, this program specializes in the treatment of children and adolescents who suffer from a variety of different mental health concerns. Some of the mental health illnesses that children/adolescents suffer from are anxiety disorders, depressive disorders, ADD (Attention Deficit Disorder), ADHD (Attention Deficit Hyperactivity Disorder), and so forth. The program is under the direction of two licensed and Board Certified Child and adolescent psychiatrists (M.D), as well as a licensed clinical psychologist (Ph.D.), licensed clinical social workers, and licensed mental health counselors.
